Preparing For Your Visit

For your first visit, plan to arrive 15 minutes early in order to register. Please fill out all forms downloaded from this site or mailed in advance to you, and bring them with you to your appointment.

To get the most out of your consultation, we suggest that you:

  • try to arrive with a full bladder, if possible. We will likely need to check a urine sample. This is particularly important on your first visit, if you have blood in the urine or are experiencing any discomfort when you urinate.
  • bring a list of all medications you are taking, including any over-the-counter medications, vitamins or dietary supplements you have taken recently.
  • bring your records with you if you are referred by another doctor or seek a second opinion. Too often, other doctors’ records arrive too late to be useful at your appointment if you do not hand carry them.
